"Everyone has a role in preventing suicide. What is your role?"
Help us create a 30 to 60 second video using this slogan and you may win one of three prizes. If you are between the ages of 14-24 and have a smart phone you can make a dfference in the lives of your friends and community by creating a suicide prevention video. The contest ends on TBA, 2017. If your entry wins, we will help you produce a professional public service announcement (PSA) at the SOU Media Center. Winning videos will be distributed in our community primarily over social media.
Winning entries will receive the following prizes:
First Prize: A $200 Amazon (C) gift certificate.
Second Prize: A $100 Amazon (C) gift certificate.
Third Prize: A $50 Amazon (C) gift certificate.

All Public Service Announcement (PSA) videos must include/use the slogan “Everyone has a role in preventing suicide. What is your role?”
Each video must conclude with the tag: “This message is brought to you by the Jackson County Suicide Prevention Coalition.”
Entries must be 30 to 60 seconds in length.
An applicant may submit more than one video.
The contest is open to all youth age 14-24 inclusive.
The first place contest winner agrees to create a suicide prevention PSA at the Southern Oregon University Media Center (SOUMC), for the Jackson County Youth Suicide Prevention Coalition (JCYSPC), for distribution on, that may include, but is not limited to, radio, television, social media,YouTube (C), the JCYSPC website and the Internet. The JCYSPC will provide resources for producing a suicide prevention PSA at the SOUMC.
By entering this contest, you are attesting that you have all of the necessary releases for all actors, voices, and/or authorizations for images, audio, music, text used in the video, for public distribution and use. Applicants shall be liable for any intellectual property right(s) or copyright(s) infringement(s) or violation(s). By entering this contest you are attesting that this video is your original work and that you grant the Jackson County Youth Suicide Prevention Coalition use your name, and any images, audio, and video in the contest video for advertising, promotion, public service, and/or other purposes without additional compensation, notification, or permission.
Entries that are felt to be in violation of intellectual property rights, inappropriate, unusable for any technical reason, or submitted after the entry window shall be, in the sole judgement of the Jackson County Youth Suicide Prevention Coalition, denied entry into the contest.
The entry window for contest videos shall be from April 5, 2017 to June 1, 2017 inclusive.
Winning entries shall be available for use/distribution by the Jackson County Youth Suicide Prevention Coalition in, but not limited to, radio, television, YouTube (C), social media, the JCSPC website and the Internet.
Winning entries will receive the following prizes:
First Prize: A $200 Amazon (C) gift certificate.
Second Prize: A $100 Amazon (C) gift certificate.
Third Prize: A $50 Amazon (C) gift certificate.
The gift certificate will be given to the winning applicant and it is the responsibility of the winning applicant to distribute the winnings appropriately to those involved in the production of the winning video.
Judging Criteria (Total 100 Points):
Originality/Creativity (30 Points)
PSA Concept (30 Points) Does the PSA clearly and accurately present the contest slogan message in a positive manner?
Communication (30 Points) Is the PSA well organized and effective?
Quality (10 Points) Is the PSA technically excellent?
Based on the Judging Criteria, three winners will be selected by the Jackson County Youth Suicide Prevention Coalition.
All entries must be accompanied by a signed application, a valid email address, and a video in HD (720p minimum) mpeg-4 (.mp4) or mpeg-2 (.mp2) format.

Once we receive a valid entry, an email will be sent to the applicant to confirm that a valid entry was received. Only valid entries are eligible for inclusion in the contest. The Jackson County Youth Suicide Prevention Coalition shall not be held responsible for lost, incomplete or invalid submissions.
By entering this contest the applicant and all those involved in the production of the contest video hereby agree to release and hold harmless the Jackson County Youth Suicide Prevention Coalition and its affiliates, officers, and members. By entering this contest, the applicant and all those involved in the production of the contest video release any rights including, but not limited to, intellectual property and distribution rights, to the video or content in the video to the Jackson County Youth Suicide Prevention Coalition.
Contest winners will be notified within 60 days of the closing of the entry window.

ACTOR AGREEMENT
This agreement is entered into by and between _____________________ (the “Actor”) and Jackson County Youth Suicide Prevention Coalition (the “Production Company”). In the event of the Actor’s death or total incapacity, “Actor” means the Actor’s heirs, devisees, beneficiaries, trusts, assignees or other successors-in-interest. “Production Company” includes the licensees, assignees, future owners, or other acquirers or successors-in-interest of the Jackson County Youth Suicide Prevention Coalition, an Oregon Domestic Nonprofit Corporation.
The Actor hereby grants to the Production Company and to its licensees, assignees, and other successors-in-interest, all rights of every kind and character, in perpetuity, in and to the Actor’s performance, appearance, likeness, name and/or voice (the “Performance”) in connection with the video production entitled ____________________________________ which is based on the original slogan “Everyone has a role in preventing suicide. What is your role?” (the “Picture”).
The Actor hereby authorizes the Production Company to photograph, videotape, film and record (on film, tape, or any other medium), the Performance and audition(s) for the Performance; to edit the same at its discretion and to include it with the performances of others and with sound effects, special effects, digital effects and music; to incorporate the same into the Picture, trailers, posters or other materials or programs related to the Picture; to use and to license others to use such records and photographs in any manner or media whatsoever, including without limitation unrestricted use for purposes of publicity, advertising and sales promotion; and to use my name, likeness, voice, biography or other information concerning me in connection with the Picture and for any other purpose associated with the Picture. The Actor further acknowledges that the Production Company owns all rights to the Picture.
The Production Company shall compensate the Actor for the Performance by payment to Actor of 0% (zero per cent) of net profits to the Production Company from all distribution and exploitation of the Picture (including Picture trademarks, service marks, logos, slogans, likenesses, names, voice, or dialogue), including royalties, dividends, or payments of any kind arising from theatre, television, video, CD, DVD, or internet distribution and exploitation. Net profits shall be determined in accordance with generally accepted accounting principles. Payment will be made to the Actor on a quarterly basis in perpetuity as the Production Company receives such monies. The Actor is responsible to pay all applicable taxes and other assessments or levies of any kind due on the receipt of income.
Unless otherwise agreed to in writing by the Production Company, the Actor will receive no compensation in the event that the Actor does not complete the Performance.
This Agreement constitutes the entire agreement by and between the Actor and the Production Company and supersedes any and all prior contracts, understandings, negotiations, and agreements with respect to the Production Company and the subject matter hereof, whether oral or written.
